St. Monica Schools will be hosting a COVID Vaccine Clinic, open to all ages 12 and up. The clinic will take place in the Grand Pavilion on Tuesday, May 25 for the first dose of the Pfizer Vaccine, and Tuesday, Jun. 15 for the second dose.
In order to schedule an appointment, you will need to complete the consent form (available for download below) and bring a copy of your insurance card and Driver's License or School ID. All forms and additional information are provided below.
